316 Health Insurance

FreedomPark's health insurance plan provides employees and their dependents access to medical insurance benefits. Employees in the following employment classifications are eligible to participate in the health insurance plan:

* Regular full-time employees (scheduled 40 hours per week) after 90 days of full-time employment

Eligible employees may participate in the health insurance plan subject to all terms and conditions of the agreement between FreedomPark and the insurance carrier.

FreedomPark will pay for health insurance coverage on the employee only, up to a maximum of $250 per month.  Any premiums in excess of $250 per month must be paid by the employee through payroll deduction.

Employees may choose to include dependents in their health coverage, and the total premium cost for the dependents must be covered by the employee through payroll deduction.

A change in employment classification that would result in loss of eligibility to participate in the health insurance plan may qualify an employee for benefits continuation under the Consolidated Omnibus Budget Reconciliation Act (COBRA). Refer to the Benefits Continuation (COBRA) policy for more information.

Information on cost of coverage will be provided in advance of enrollment to eligible employees. Contact the Administrative Manager for more information about health insurance benefits.
